The standard form of a circle is [tex](x-h)^2+(y-k)^2=r^2[/tex].  We will fill in our h, k and r accordingly.  [tex](x-(-5))^2+(y- \frac{1}{2})^2=1^2 [/tex]  which simplifies down to  [tex](x+5)^2+(y- \frac{1}{2})^2=1 [/tex]  choice C.  There you go!
